• Annual Activity Report (AAR) is generated through Works. Please follow the SCSE AAR guidelines.
• Form 25: Appraisals of Probationary Faculty or  Form 25 editable version - evaluation of tenure-track faculty done once every year
• Teaching evaluation agreement forms are due September 15. These are department-specific and have been provided to your Department Head.

Performance appraisals are to be completed by February 15, 2026 for the following groups Civil Service, AFSCME, Teamsters, and Academic P&A, including Post-Docs. The appraisals will again be completed using the Performance Appraisal Tool (PAT)
