Required Forms for Registrants
Every participant needs to complete health and liability forms and submit them in advance or upon arrival at the conference.
The photo release form included is optional. The liability release form for the Recreational Sports Outdoor Program is also optional, but necessary if LSYS participants would like to utilize the climbing wall.Â

Payment
Payment for symposium registration is due by May 1, 2019.

Cancellation Policy
If a registrant cancels after May 3rd, we will only be able to reimburse 50% or half of the individual's registration costs. Keep in mind, that registrants are welcome to find another individual to attend in their place as long as the person is in grades 7-12.
