Use this form if you are a:

• SCSE student declaring or changing a SCSE major or sub-plan associated with your major
• SCSE student adding or dropping a second SCSE major of the same degree type (e.g. current B.S. in Mathematics, adding B.S. in Chemistry)
• SCSE student who is already a dual degree (two degree types) and adding or dropping another SCSE major of the same degree type (e.g. current B.S. in Biology and B.A. in Criminology, adding a B.A. in Chemistry)
• Student in CAHSS, CEHSP or LSBE adding or dropping a second major of the same degree type in SCSE (e.g. current B.A. in Sociology, adding B.A. in Computer Science)

Dual Degree Program Application

Use this form if you are a:

• SCSE student adding or dropping a second degree type in SCSE (e.g. current B.S. in Biochemistry, adding B.A. in Chemistry)
• SCSE student adding or dropping a second degree type in CAHSS, CEHSP or LSBE (e.g. current B.S.M.E. in Mechanical Engineering, adding B.B.A. in Management)
• Student in CAHSS, CEHSP or LSBE adding or dropping a second degree type in SCSE (e.g. current B.A.Sc. in Psychology, adding B.S. in Biology)
